Getting started is simpler than you think.
Check who qualifies for HCBS-AMH services, then send a referral. We'll guide you through the rest.
Who is eligible
HCBS-AMH services are for adults who:
- Are 18 years of age or older
- Have an eligible mental health diagnosis
- Are Medicaid eligible or actively enrolled
- Have a functional need for home and community-based supports
- Meet the HCBS level-of-care requirements
- Take part in person-centered planning
Eligibility is determined through state or managed care authorization processes. Not sure if you qualify? Reach out anyway. We'll help you find out, and point you to other resources if this isn't the right fit.

We reach out to learn more, answer questions and explain options in plain language.

Authorization
We coordinate with the state or MCO and help gather what's needed for approval.
